Terms and Conditions / Cancellation Policy:
A place cannot be confirmed on a course until an enrolment form is received and any payment(s) due have been made. Furness College reserves
the right to cancel courses and alter class times. Where a course is cancelled by Furness College or times/days altered to such an extent
that your attendance becomes impossible you will receive a full refund or the fees will be rolled over onto the next course at your discretion.
Information detailed on this enrolment form concerning Guided Learning Hours may be made available to the Benefits Agency (on written
request) and may constitute part of your entitlement to benefit (JSA). Your sponsor may be informed of your progress and/or attendance.
No fee will be issued if you opt to transfer to a later course up to 12 working days before the course commences. A 50% refund will be issued
in the event of any cancellation/transfer being received less than 12 working days before the course commences; No refund will be issued
in the event of a cancellation or transfer being received 7 working days before commencement. None attendance without prior notification
will result in no refund being issued. For all cancellations/transfers additional fees will apply for any examination entry/registration costs
associated with your booking. Cancellations must be made in writing to our email address; firstname.lastname@example.org or to the Business
& Employment Support Team, Channelside, Barrow-In-Furness, Cumbria, LA14 2PJ. We recommend that you send cancellations via recorded
delivery and retain the proof of posting. Failure to pay any due fees may result in Furness College with-holding certificates/qualifications until
payment is received. Failure to attend for funded training without prior notification may result in you being invoiced for the cost of the course/
exam registration fees incurred. All information will be processed and where necessary held in accordance with the Data Protection Act 1998.
The provision of some of the information in this questionnaire constitutes sensitive and personal data as defined within the Data Protection Act
1998 and as such requires your specific permission. Your signature to this document will be deemed to be such specific permission.
How We Use Your Personal Information:
The personal information you provide is passed to the Chief Executive of Skills Funding ("the agency") and, when needed, the Young People's
Learning Agency for England ("the YPLA") to meet legal duties under the Apprenticeships, Skills, Children and Learning Act 2009, and for the
Agency's Learning Records Service (LRS) to create and maintain a unique learner number (ULN). The information you provide may be shared
with other partner organisations for purposes related to education or training.

Candidates sponsored by an employer/external organisation hereby consent to the sponsor being given details of progress, attendance and
achievement. I have enrolled on the above course with appropriate guidance from Furness College Business & Support Team on the following
